Testimonial organiser Heather Wilkanowski said: "We have been very fortunate that Peter Reid has agreed to bring a club as big as Leeds for John's game and I know that many supporters are looking forward to it as much as John himself.
"I have been working towards finding a suitable date with Leeds since the end of last season but this has always been a preference for Peter Reid because of the break in Premiership games."
The testimonial fund will be making donations to Macmillan Cancer Nurses as well as the club's Youth Development Department.
Ticket prices on Tuesday 11th November (7:45pm) will be;
£15 seating, £12 terracing and £5 for concessions.
Leeds reserves were due to play away to Everton that night and this will now be played on a new date to be arranged.
